Outreach Instructors
Please note that as of November 1, 2011 the OSHA OTIEC is charging a fee of $5 for new cards and a fee of $25 for replacement cards. All request cards received on or after November 1, 2011 must be accompanied with payment. We take credit cards, checks, money orders, I’m sorry we don’t take cash and personnel checks. Failure to include an acceptable means of payment will result in your request being returned to you and delay the receipt of the requested training cards. Please keep in mind we have 30-days to process from date of receipt of properly completed paperwork. It is your responsibility as an Authorized Outreach Trainers to make sure all requirements are effective as July 1, 2011, are met along with required payment to have an acceptable request.
We have also received a directive from OSHA requiring the OTIEC’s to conduct audits and training site visits beginning in October 1, 2011. The OTIEC is required to conduct the audits and visits for the following reasons: complaints, repeated errors on paperwork and random audits. We may request a copy of your upcoming training schedule, which you are required to provide, so that an official from the OTIEC may observe your training session.
Requesting New and Replacement Card forms
For students who have lost their 10/30hr card contact your trainer for replacement. Trainers to request a replacement for one of your students who have completed a course fill out and include appropriate paperwork along with replacement fee and send to the OTI.
Replacement Card Information
Replacement requests are accepted via mail or e-mail (preferred method). Please submit a written request for a replacement card including: the reason for replacement; individual needing replacement; a copy of the original card request and the payment form.
Note: if you are inquiring about a replacement card due to damage or misspelling, you need to submit all of the above information and the original card.
When requesting cards please submit the following forms:

Ways to submit your request
To submit your requests –E-mail or Mail
Student Card Requests can be submitted via e-mail (preferred method) or mail. Outreach Trainers must send requests for student cards to the OSHA Training Institute Education Center where they took their last trainer course (OSHA 500, 501, 502 or 503) Please note, under the new guidelines, Outreach Trainers are obligated to send a complete and error-free Program Report to the OSHA Training Institute within thirty (30) calendar days from the “End Date” notated in Box #12 of the Program Report. Sending incomplete and/or incorrect documentation will detract from the thirty day window an Outreach Trainer is given. Outreach Training card requests could take up to thirty (30) calendar days to process from the date that an error-free Program Report is received by the OSHA Training Institute.
